Individual Counseling / Ages 12+

Utilize various methods for lifelong development in mental, emotional, physical, social, moral, educational, career, and spiritual aspects. For patients younger than 12 years of age, Mr. Chavez will consult with the parent/legal guardian beforehand to determine if telehealth would be effective for the individual.

Addiction Counseling / Ages 17+

Use various methods, including interpersonal, cognitive, cognitive-behavioral, behavioral, psychodynamic, affective, and 12-step approaches, to help individuals achieve abstinence from addictive substances and behaviors.

Family Counseling

Use various methods, including interpersonal, cognitive, behavioral, psychodynamic, affective, and family systems approaches, to support lifelong development and adjustment in mental, emotional, physical, moral, social, educational, spiritual, and career aspects within families.

Marriage and Couples Counseling

The main goal is to address challenges in the dynamics of adults living as couples by identifying, analyzing, and mitigating issues. Our approach encourages open dialogue, constructive problem-solving, and holistic understanding to foster a harmonious and thriving partnership.

Referrals

The experienced Licensed Professional Counselor (LPC) will diligently identify individuals in need of comprehensive psychiatric services, ensuring prompt interventions and treatments for improved well-being and enhanced quality of life.

Emotional Support Animal Assistance

The fee for the completion of an Emotional Support Animal (ESA) Letter is set at $300.00, and this amount is non-negotiable. Please note that Mr. Chavez retains the right to refuse service if he considers the animal may pose a risk to others. Mr. Chavez is not obligated to provide an explanation for refusal.

Crisis Counseling / Ages 18+

Short term counseling interventions to address immediate situations including factors such as safety and immediate needs. ONLY SELF PAY

Consulting

Provide assistance in understanding and solving current or potential problems that the consultee may have in relation to a third party, whether individuals, groups, or organizations. ONLY SELF PAY

Chemical Dependency Evaluations

The fee for a behavioral health evaluation is a fixed, non-negotiable rate of $300.00, payable prior to the completion of services.

Mr. Jesus Chavez, LPC-S, LCDC-CCS, provides evaluation services only. His professional involvement is limited to conducting the evaluation and providing a written report, when applicable. Court testimony, depositions, expert witness services, legal consultation, and court appearances are not included in the evaluation fee and are not routinely offered as part of his professional services.

Should a valid subpoena, court order, or other legal requirement necessitate participation beyond the evaluation process, any such involvement shall be addressed separately and may be subject to additional fees, consistent with applicable laws, regulations, and professional standards.


Jesus Chavez, LPC-S, LCDC-CCS, provides transparent pricing for FMLA documents and related forms. The base rate is $50.00 for up to 3 pages, with an additional charge of $5.00 for each additional page. This straightforward pricing structure ensures fairness and clarity, allowing clients to easily understand the costs associated with comprehensive counseling support.
